Three kinds of options: numbing products and condoms act tonight, prescription medication works while you take it, and training (feeling the point of no return, calming the body, the pelvic floor) is slower but changes the reflex itself.
There are about ten real options, and they don’t work on the same timescale. Some act tonight and stop when you stop. Others take weeks and change the reflex itself. Here they are side by side, then one by one.
The options at a glance
| Option | How fast it acts | Evidence | Downside |
|---|---|---|---|
| Numbing sprays and creams | Minutes | Good: first-line for urologists | Less sensation, can numb your partner, only while you use it |
| Delay condoms | Tonight | Modest, studied in men without premature ejaculation | Small gain |
| Prescription pills | Hours (on demand) or weeks (daily) | Good | Side effects, needs a doctor, only while you take them |
| Treating erection problems first | Depends on the cause | Recommended by urologists when both are present | Only if erections are part of it |
| Calming anxiety, slow breathing | Gradual | Calm helps; breathing alone not shown to delay ejaculation | Rarely enough on its own |
| Learning the point of no return | Weeks | Weak when used alone as a trick | Takes regular practice |
| Pelvic floor training | Weeks | Encouraging, small studies | Takes time, needs to include relaxing too |
| Changing your habits | Weeks to months | My own view, not tested | Slow |
| Distraction tricks | In the moment | None that it lasts | You enjoy it less, nothing changes |
| Supplements and alcohol | Varies | No good evidence | Hidden drugs in some supplements; alcohol weakens erections |
First, check the cause
If it started suddenly after years without trouble, or if you also have erection trouble, pain, or urinary symptoms, see a doctor before anything else. A physical cause (an erection problem, the thyroid, the prostate, a medication) changes everything, and dealing with it often helps a lot.
The ten options, one by one
1. Numbing sprays and creams
They contain a local anaesthetic and dull sensation, and the American urologists list them among the first options. They work for a lot of men, with a cost: less sensation, sometimes softer erections, and numbness that can reach your partner. See do delay sprays and numbing creams work.
2. Delay condoms
A condom dulls sensation a little on its own, and “delay” condoms add a small amount of numbing agent. Useful as a helper, but the gain is modest. More in do condoms help you last longer.
3. Prescription pills
Some antidepressants, prescribed by a doctor, delay ejaculation, taken daily or before sex. Dapoxetine is the only one designed for it, and it isn’t sold in the US. See do premature ejaculation pills work.
4. Treating erection problems first
When both problems are there, the urologists say to deal with the erection first: a man who struggles to stay hard often finishes sooner. Viagra doesn’t help timing on its own. See does Viagra help premature ejaculation.
5. Calming anxiety and breathing
Anxiety and tension are among the most cited factors, and a body on alert goes faster. Slow breathing helps you stay calm, even if it hasn’t been shown to delay ejaculation on its own. See can anxiety cause premature ejaculation.
6. Learning the point of no return
Ejaculation has a moment after which you can’t hold it back. Learning to feel it coming early enough is the idea behind stop-start. Used alone as a trick, the evidence is weak; as something you train, it’s at the heart of what worked for me. See does the stop-start technique work.
7. Pelvic floor training
The muscles under the pelvis take part in ejaculation. A small 2014 study found clear progress after 12 weeks of training. Strength is only half of it: those muscles also need to be able to let go. See do Kegel exercises help premature ejaculation.
8. Changing your habits
The way I see it, years of finishing fast teach the body that fast is normal, and habits can be retrained. That’s my view, not a study. See does masturbation cause premature ejaculation.
9. Distraction tricks
Counting, thinking about something unsexy: I tried those, and nothing lasted. They take your attention away from exactly what you need to learn to read. See why most advice for premature ejaculation fails.
10. Supplements and alcohol
No good evidence for supplements, and some sexual enhancement products have been found to contain hidden drugs. Alcohol can dull sensation, but it also weakens erections. See do supplements like zinc help and does alcohol make you last longer.
What I’d do
Options 1 to 4 change what happens tonight or while you use them. Options 5 to 8 are the slow road, and the one I took. Being able to choose when I come, instead of having it happen to me, is hugely rewarding. How to train those levers together, and in what order, is what the program is for, so I won’t turn this page into a list of exercises.
